The drug did the work. Now keep it off.
The thing you dread is watching it come back. In one semaglutide trial, people who stopped regained about two-thirds of what they had lost within a year.1 That is not a willpower problem. It is what happens without a plan for after. OffRamp turns the plan into a daily routine your iPhone runs for you.
Regain is not a character flaw. It is the default.
A GLP-1 turns the food noise down and the appetite with it. Stop, and both come back. The muscle that went with the fat does not come back on its own, unless you eat and train for it. The trials that followed people after stopping are blunt about what happens next.
In the STEP 1 extension study, participants regained about two-thirds of the weight they had lost within a year of stopping semaglutide.1
Tirzepatide tells the same story. In SURMOUNT-4, people switched from tirzepatide to placebo regained a substantial share of their lost weight over the following year.2
What keeps it off is not a secret. It is a routine.
Enough protein, spread across meals, is the lever that decides whether what you lose is fat or muscle.
Lifting is what gives the protein somewhere to go. It is how you keep the muscle that keeps your metabolism honest.
Watch the four-week average. A rising average is the earliest hard signal that maintenance has slipped.
Coming off is a conversation, not a cliff. Bring a plan, your dose log and your side-effect history to the person who prescribed it.
None of this is new. The hard part is doing it every day, for a year, while the noise comes back. That is the job OffRamp does.
Photograph a meal. OffRamp does the rest.
Every piece of the routine is either automatic or one tap away, and every number tells you where it came from.
Protein-first photo logging
Point the camera at the plate. Get a protein estimate you can correct in one tap. Protein first, everything else second.
A lifting plan sized to your floor
A resistance-training plan built around the protein and calories you are actually eating, with a session player and a rest timer that keeps running in your pocket.
Dose and injection-site log
One tap to log the shot. A rotation map so you do not reuse the same spot. A refill countdown that reminds you before the box runs out.
Free, foreverSide-effect diary
Log how you feel, even on the good days. See symptoms lined up against your dose history, so you can show your prescriber a pattern instead of a memory.
Weekly regain-risk score
Every Sunday, one number from your weight trend, steps, sessions, protein and food noise, with a breakdown of what moved it. An estimate, and it says so.
Food-noise check-ins
Thirty seconds, once a week, as the appetite returns. It usually gets loud before the scale moves, which is exactly why you track it.
Taper discussion plan
A calendar with every dose left blank, plus your logs and the questions worth asking. You hand it to your prescriber. They fill in the amounts.
Apple Health and Apple Watch
Weight trend from Apple Health, not one noisy morning. Steps and workouts from your Watch. Log a weight or start a session from your wrist.
Live Activity, widgets, on-device coaching
Protein left today on your Lock Screen and in the Dynamic Island. A daily read written on your iPhone with Apple's on-device models on iOS 26, and by OffRamp's servers on earlier versions.
Wherever you are in it, there is a mode for it.
Lose fat, not muscle.
On the drug mode- Hit your protein. Photo-log a meal in five seconds and watch the ring close.
- Never miss a shot. Dose reminders, a rotation map and a refill countdown.
- See side effects against dose history, ready to show your prescriber.
- Keep the muscle you have with a lifting plan sized to what you eat.
- Watch the real trend from Apple Health, not one morning's number.
Keep it off after you stop.
Off-ramp mode- A taper discussion plan to hand your prescriber, doses left blank for them.
- A weekly regain-risk score from your weight trend, steps, sessions and protein.
- Food-noise check-ins as the appetite comes back.
- Maintenance protein and calorie targets for the version of you that is not on the drug.
- An early word when your four-week weight trend turns, while it is still small.
Switch modes in Settings whenever you and your prescriber decide it is time. Same account, same logs, same subscription.
Every number says where it came from.
Measured means Apple Health or you typed it. Estimate means a model produced it, and you can read exactly how.
OffRamp logs what you took and never suggests a different amount. Every dose decision belongs to you and the clinician who prescribed it.
No ad SDK, no tracking prompt, ever. Your Apple Health data is never used for advertising and never sold.
Meal photos are analysed by an AI service you consent to first and deleted from our servers 30 days after upload. Delete your account from inside the app in two taps and everything goes.
One subscription. Both modes. Start with a free week.

Coaching, not medical advice
OffRamp is a coaching and tracking app, not a medical device. It does not diagnose, and it never prescribes, dispenses or changes medication. Any change to your dose is a decision for you and your prescriber. For adults 18 and over.
The things people ask before they join.
Is OffRamp medical advice?
No. OffRamp is a coaching and tracking app. It does not diagnose, it will not tell you whether a symptom is serious, and it never tells you what dose to take. It records the dose your prescriber gave you and helps you build a plan to take to your prescriber. Every change to a GLP-1 dose is a decision for you and the clinician who prescribed it. If a symptom looks like one people are told to report, OffRamp says so and tells you to contact your prescriber.
Does it work with my medication?
OffRamp works alongside whichever GLP-1 medication your prescriber gave you, injectable or oral. It logs what you take. It does not sell, source or link to medication, and it has no opinion about which one you should be on.
Do I need an Apple Watch?
No. An iPhone on iOS 17 or later is enough. Weight, steps and workouts come from Apple Health, and you can type a weight in. The Watch app adds logging weight and starting a session from your wrist, plus complications. On iOS 26 the daily coaching is written on your iPhone; on earlier versions OffRamp's servers write it.
What does it cost?
Seven days free, then $59.99 a year. Or $12.99 a month without a trial. Both are App Store subscriptions you cancel any time in your Apple Account settings. The dose log, reminders, weight chart and side-effect log stay free without a subscription.
When does it launch?
Soon, on the App Store for iPhone. We do not publish a date until Apple has approved the build. Join the waitlist and you get the email the day it ships, before anyone else hears about it.
What does the regain-risk score actually measure?
It weighs eight things from your own logs: weight trend, weight swing, protein, strength sessions, steps, food noise, weeks since your last dose and where you are in your taper. It adds them up to a number from 0 to 100, refreshed every Sunday. It is an estimate produced by software from the data you logged. It is not a medical assessment, it is not a prediction about your health, and no clinician has reviewed your case.
What happens to my data?
Your Apple Health data is never used for advertising or marketing, and never sold. There is no advertising SDK and no tracking prompt. Meal photos are analysed by an AI service you consent to first, and deleted from our servers 30 days after upload; the meal and its numbers stay in your log. Coaching on iOS 26 is generated on your iPhone, so those logs never leave the device for it. Delete your account from inside the app in two taps and everything goes.
Be first when it ships.
OffRamp is coming to the App Store for iPhone and Apple Watch. The waitlist gets the launch email first. No fake countdown, no fake spots left, just the email on the day.
